Note: This calculator follows ACR TI-RADS 2017 and Soto's TI-RADS+ scoring rules.
- TR5: Highly suspicious — consider FNA ≥1.0 cm, follow-up at 1 year if smaller.
- TR4: Moderately suspicious — consider FNA ≥1.5 cm, follow-up at 1, 2, 3, and 5 years if smaller.
- TR3: Mildly suspicious — consider FNA ≥2.5 cm, follow-up at 1, 3, and 5 years.
- TR2 & TR1: No routine FNA, repeat ultrasound only if clinical concern arises.
